Patient Forms, Map and Directions to Columbia Office

Your first visit to Ogden Dental establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, including your medical history, and give you time to get to know the doctor. To understand what to expect for your first visit to our practice, please read through this page. You'll find all the practical information you need, such as a map and directions to our office, practice hours, payment policies and more. There's also background information about our first visit procedures. You can even save some time at your first visit by printing out and completing the patient forms in advance of your appointment.

Patient Forms

Please print and fill out these forms so we can expedite your first visit:

In order to view or print these forms you will need Adobe Acrobat Reader installed. Click here to download it.

Being well-prepared for your appointment will ensure that the doctor has all of the needed information to provide the best possible care for you. It also will help relieve any unnecessary anxiety you may be feeling.

Please bring a list of all concerns and questions you may have for the doctor. It's all too easy to forget something during the process of getting acquainted with the office, the staff, and the doctor.

If you take more than three medications on a daily basis, please bring a list of your medications (including over-the-counter) so we may make a copy. It's much more efficient and accurate to prepare this in advance.

If you expect us to use radiographs (x-rays) or digital radiographic images from another dental office, please make sure that all necessary arrangements have been made to have them transferred (which may include transmitting signed consent) prior to your appointment time. It is difficult to give you a complete "report card" when we don't have all the information we need.

What To Expect

There are essentially three ways to enter our practice: 1. As a single-problem or emergency patient, 2. As a new patient seeking optimum dental health and a "dental home", or 3. As a TMD patient (usually by referral).

For single-problem or emergency patients, our purpose is to identify your problem and suggest the best method of dealing with it. This will usually involve a brief examination and appropriate x-ray(s). Actual treatment can rarely be performed with the limited amount of time scheduled for this type of visit, but we'll be sure we're doing our best to alleviate pain and make any necessary treatment arrangements as soon as possible.

If you're entering as a TMD patient, your appointment will occur on a Thursday morning. (See "TMJ Disorders" on our home page.)

If you're seeing us as a new patient for continuing dental care, the following will occur:

A staff member will review your dental history and any concerns you have at the present time. Then, you'll meet Dr. Ogden, who will perform the following:

Examination of all the soft tissues that line the inside of your mouth, including the visible portion of your throat, and the top and bottom of your tongue, for any evidence of early cancer or other abnormality. And we'll show you how to clean the surface of your tongue with a simple instrument you will take home with you.

Complete clinical examination of the gums and any teeth which may be loose. We'll also make sure you know exactly how to use floss effectively, and how to brush your teeth effectively, along with any other aids which might suit your particular situation.

Evaluation of your bite and the function of your jaw joints (TMJ's).

A photographic and visual examination of your teeth.

We'll then make radiographic images (x-rays) sufficient to make sure we've covered all the bases. Afterward, we'll discuss what, if anything, we recommend to provide you with the level of care you're seeking.

In most cases, your first professional cleaning will be a part of your treatment plan and will not be performed at the initial visit. If there are needs other than the cleaning, we can usually coordinate some or all of them with the cleaning visit.

Driving Directions

Our office is conveniently located near the intersection of Highway 63 and Broadway (Route WW).

At the first traffic signal on Broadway east of Highway 63, turn north onto Keene St. We are the fourth building on the right, just before you reach the intersection with St. Charles Road. We are directly across from the new Columbia Orthopedic Group complex. If you are traveling south on Keene, we are the first building on your left after you pass St. Charles Road.
